What to Bring With You
We ask that you bring your insurance card and photo ID with you to every visit. If you have previous medical records, please bring them as well or have them mailed to our office.
Your Registration Process
Please plan to arrive to your appointment 15 minutes early to complete the registration process. You may download our registration forms below. Please complete them and bring them with you. These forms are also available in the office. You will also be asked to sign our HIPAA Acknowledgement, as required by law, so that you are aware of our privacy policies.
Your Payment
Please be prepared to pay your deductibles, co-pays, co-insurance or any other amounts due at the time of your visit. We accept a variety of payment options including Cash, MasterCard, Visa or American Express.
